Ohio Masonic Home has decided to remain with Aetna for 2018 and offer two high deductible health plans, similar to two of your plans offered in 2017. Please refer to the Aetna certificate of coverage for more details.

Spousal Insurance Form

If your spouse is employed and is offered coverage through their employer, they are not eligible for medical coverage with The Ohio Masonic Home.

If your spouse is paid a fringe benefit by their employer in lieu of health care so that they can purchase health care on their own, they are not eligible for medical coverage with The Ohio Masonic Home.

If your spouse qualifies for coverage with The Ohio Masonic Home and you plan to include your spouse on OMH coverage, you must complete the waiver and submit any required proof.

An additional spouse fee will be deducted each pay period for all employees who have a spouse on the plan.

THE OHIO MASONIC HOME WELLNESS PROGRAM

Ohio Masonic Home cares about the health and welfare of our employees. Receiving preventive care not only helps you know your health numbers, but makes it possible to catch life threatening conditions early on. 26% of participants were identified as ‘at risk’ or ‘critical‘ for a medical condition.

Employer Health Savings Account contributions are provided to those who received their annual primary care physician physical and biometric screening.

Please note: spouse participation is required to receive the HSA employer contribution if they are on the medical plan.
