Unum’s Group Critical Illness insurance can help protect your finances from the expense of a serious health problem, such as a stroke, heart attack, or cancer.
Advantages of the plan
- Guaranteed Issue is available during this enrollment only up to $20,000 for employee and $10,000 for spouse. If you previously waived coverage or currently have coverage and would like to increase it no medical exam or health questions are required.
- Children are automatically included at 25% of the employee benefit, and are covered for additional childhood conditions, such as cerebral palsy, cleft lip or palate, cystic fibrosis, Down syndrome and spina bifida.
- Employees currently enrolled with UNUM can increase coverage up to guaranteed maximum.
- Employees that waive coverage today can enroll up to guaranteed issue maximums.
Eligibility
- Employees enrolled in the traditional medical plans
- Employees enrolled in the HSA Advantage Medical Plan
- You can elect in addition to the CCHMC-provided $5,000 policy up to the guaranteed issue maximums listed above!
- Employees who are benefits eligible and waive the CCHMC medical plans.
How it works:
The benefit is paid to the employee and can be used for medical out of pocket or any other expenses. If an employee is enrolled in both the employer paid (free when enrolled in the HSA plan) and voluntary critical illness plans, you receive a wellness benefit of $75 for each plan per covered person which is $150 per person per calendar year when you have completed a covered wellness test.
Covered illnesses:
Cancer, heart attack, stroke, major organ failure, end stage renal failure, blindness, brain tumor. Additional Child Conditions: cleft palate, cerebral palsy, cystic fibrosis and more.
